By checking here, I confirm my organization's decision to enroll in ExpressAssessment service which entitles us an annual checkup of the way our Center uses our SourceMedical software. I am authorized to enroll my organization for ExpressAssessment and I understand that I will be invoiced on an annual basis. The initial subscription is for a one year period beginning today, with automatic renewal for another year upon the anniversary date for start of service.
